Quality Profiles explores the implications of quality improvement (QI), the rationale for change, and the barriers to improvements in the context of an expanded discussion of the personal, social, and economic impact of the health care conditions under review. The overall goal of this series is to provide health care organizations and employers with a rich, up-to-date collection of QI resources, illustrated by case studies, to use as a foundation for new program development.

The case studies featured demonstrate multiple ways to conduct successful quality initiatives. We have used a format designed to make the profiles easy to understand, adapt and implement. In addition to the full profiles, additional case studies and examples are highlighted as Quality Lessons and Quality Options. |

Many of the plans agreed to share templates of the tools
they used to help implement their initiatives. Examples
include patient education brochures, focus group questions
and provider information letters. |
